Thank you for considering a gift to the Feminist Women's Health Center. There are a number of ways that your gift can be made.

By Phone

Please call (404) 248-5445 and let us know how you would like to contribute. We accept Mastercard, VISA, American Express and Discover cards.

By Mail

You can also send your gift by mail by writing check to the Feminist Women's Health Center or by including your credit card information and sending it to:

For your convenience, we have included a printable form that you can fill-in online and then print to send along with your check or credit card information. (You will need Adobe Acrobat 5 or higher to use the form.)

If you are mailing a credit card gift, please include the following:

• the type of credit card -- Mastercard, VISA, American Express or Discover
• your name as it appears on the card
• the credit card number/account
• the expiration date
• the donation amount
• the 4 or 3 digit security code on the back of your card (if any)
• your complete billing address and phone number.
